India's massive general election 2009

On May 16th, 2009, the Election Commission of India announced the results of its recent month-long India-wide election for their lower house of Parliament - the largest democratic election in the world. An estimated 714 million voters (from a population of 1.2 billion) were eligible to cast their vote in one of five separate phases at over 800,000 polling stations, starting on April 16th. Logistically difficult, massive in scale, and opposed by various rebel groups, separatists and protestors, the elections still managed to be held with minimal disruption, with an average voter turnout of greater than 56%. The big winner was the the Indian National Congress party, which will form the new government under the incumbent prime minister Manmohan Singh. As with any photo story from India, it is impossible to capture every aspect in just a handful of pictures - collected here are only some of the scenes that played out across the nation over the last month. กลุ่มโรคการกดทับเส้นประสาทบริเวณข้อมือ Carpal tunnel syndrome (CTS)

เป็น กลุ่มโรกกดทับเส้นประสาทที่พบได้บ่อยที่สุด สาเหตุเกิดจากมีการกดทับเส้นประสาท median nerve ที่บริเวณข้อมือซึ่งบริเวณนี้จะมีลักษณะเป็นโพรงหรืออุโมงที่ประกอบไปด้วย ผนังด้านหน้าได้แก่เอ็นที่เรียกว่า tranverse carpal ligament ส่วนผนังด้านหลังได้แก่กระดูกข้อมือ ถ้าโพรงนี้ตีบแคบจากสาเหตุใดก็ตาม ก็จะทำให้เกิดการกดทับเส้นประสาท median nerve ทำให้เกิดอาการปวด ชาบริเวณนิ้วหัวแม่มือถึงนิ้วนาง

เส้นประสาม median nerve ทำหน้าที่รับความรู้สึก และการเคลื่อนไหวบริเวณน้ิหัวแม่มือ นิ้วชี้ นิ้วกลาง และนิ้วนาง

สาเหตุ

สาเหตุสำคัญของโรคนี้คือการใช้ข้อมือ และมือซ้ำๆ ตัวอย่างสาเหตุที่พบว่าเกิดบ่อยที่สุดคือการคีย์คอมพิวเตอร์ ตัวอย่างอาชีพที่พบโรคนี้ได้บ่อย

การเขียนหนังสือ
การใช้เครื่องมือที่สั่น เช่นเครื่องเจาะถนน
กีฬาบางประเภท

ส่วนใหญ่ไม่ทราบสาเหตุของโรคนี้ แต่มักจะพบโรคนี้ร่วมกับภาวะดังต่อไปนี้

โรคอ้วน
การตั้งครรภ์
ต่อมไทรอยด์ทำงานน้อย
อาจจะเกิดตามหลังข้ออักเสบ เช่น วัณโรค โรครูมาตอยด์ โรคเก๊าท์
โรคเบาหวาน
ได้รับอุบัติเหตุที่ข้อมือ
เอ็นอักเสบจากการใช้งานอย่างต่อเนื่อง เช่น การพิมพ์ดีด การบิดผ้า

อาการของกลุ่มโรคการกดทับเส้นประสาทบริเวณข้อมือ

ผู้ป่วยจะมาด้วย ชาเป็นเหน็บ หรือบางท่านจะมีอาการปวด แสบร้อนบริเวณฝ่ามือ นิ้วชี้ นิ้วกลาง ด้านหน้าหรือด้านหลังผ่ามือ มักจะเกิดตอนกลางคืนในขณะหลับ ทำให้ต้องตื่นนอนเพื่อสะบัดมือ หรือนวดฝ่ามือเพื่อช่วยให้เหน็บชาดีขึ้น
ผู้ป่วยบางท่านจะมาด้วยฝ่ามือ นิ้วมือ เหน็บชาและอ่อนแรงในขณะขับรถ หรือขณะถือหนังสือพิมพ์ ทำให้ต้องลดมือลง
อาการปวดอาจจะร้าวไปถึงข้อศอก
กล้ามเนื้อบริเวณโคนนิ้วหัวแม่มือฝ่อ
ไม่มีแรงกำ

การตรวจร่างกายอย่างง่ายๆ

การตรวจร่างกายอย่างง่ายๆที่เรียกว่า Tinel's test โดยการเคาะบริเวณตรงกลางข้อมือซึ่งเป็นบริเวณที่เส้นประสาท median nerve ผ่าน จะมีอาการปวดหรือลักษณะไฟช็อต ร้าวไปที่นิ้วมือ การทดสอบนี้มีความไว40-60% มีความจำเพาะ 70-94%
การตรวจ Phalen's test คือให้งอข้อมือ 90 องศาเป็นเวลา 1 นาที ผู้ป่วยจะมีอาการชา หรืออาการปวดบริเวณฝ่ามือและนิ้วมือ
ในกรณีที่เป็นรุนแรงและเป็นมานานจะพบกล้ามเนื้อบริเวณโคนนิ้วจะฝ่อลีบและอ่อนแรง
การตรวจ Durkan 's test โดยการกดบริเวณข้อมือเป็นเวลา 30 นาที หากมีอาการชาหรือปวดจะแสดงว่ามีการกดทับของเส้นประสาท
การตรวจการรับความรู้สึกที่
การตรวจคลื่นไฟฟ้าของเส้นประสาท median nerve จะทำในกรณีที่อาการและอาการแสดงไม่ชัดเจน

การรักษา
การรักษาโรคกลุ่มโรคการกดทับเส้นประสาทบริเวณข้อมือ ควรจะรีบให้การรักษาตั้งแต่เริ่มเป็น โดยการให้พักข้อมือ หลีกเลี่ยงกิจกรรมที่ทำให้ปวดมากขึ้น การใส่ splint เพื่อพักข้อมือ หากมีอาการบวมให้ประคบเย็น การรักษาแบ่งออกเป็น

การรักษาโดยไม่ผ่าตัด

การใช้เผือกอ่อนดามข้อมือ
การใช้ยา ยาที่นิยมใช้คือยากลุ่ม NSAID ซึ่งเป็นยาแก้ปวดแก้อักเสบ เช่น aspirin, ibuprofen จะสามารถลดอาการปวดและอาการบวม
การใช้ยา steroid จะใช้รักษาในช่วงระยะเวลาไม่นาน การใช้ยานี้ควรจะอยู่ในการดูแลของแพทย์
การปรับเปลี่ยนพฤติกรรมที่จะใช้ข้อมือเพื่อลดการอักเสบของข้อ มือ การทำกายภาพบำบัด เช่น การทำอุลตร้าซาวด์ การบริหารมือ ซึ่งจะได้ผลดีในผู้ที่เริ่มต้นมีอาการไม่มาก
การฉีดยา steroid เข้าบริเวณข้อมือพบว่าได้ผลร้อยละ 80 และต้องแก้ไขสาเหตุ

การผ่าตัด

การผ่าตัดจะกระทำเมื่อ

การรักษาด้วยยาและปรับเปลี่ยนพฤติกรรมแล้วไม่ได้ผล
ในรายที่มีอาการรุนแรงกล้ามเนื้อฝ่อมักจะไม่ได้ผลเมื่อรักษาด้วยยา

วิธีการผ่าตัด

การผ่าตัดโดยการเปิดแผลและตัดเอ็นที่ด้านหน้า และขยายช่องให้ใหญ่ขึ้น
การผ่าตัดโดยการส่องกล้องข้อดีแผลจะเล็กกว่าการผ่าตัด
